BDI, BAI, and Symptom Inventories on the NCMHCE: What They Measure and How to Read a Score

The Beck Depression Inventory, the Beck Anxiety Inventory, the PHQ, and the AUDIT are on NBCC’s list of the tools counselors use most, and “use screening instruments” and “determine your competency for administering assessments” are exam tasks[5]. The items test which inventory measures what[1][2][3][4], how to read a score, what a screen can and cannot do, and the competence and cultural rules around administration. Reading and using scores
- Severity, not diagnosis: an inventory quantifies symptoms over a window; diagnosis is the clinical interview against DSM-5-TR criteria.
- Baseline and change: administer at intake and at intervals; treatment-plan progress review (Domain 3B) uses the change scores.
- Critical items: any endorsement of a suicide item (PHQ-9 item 9, BDI-II item 9) triggers a direct risk assessment regardless of total score.
- Sensitivity vs. specificity: screens are tuned to catch cases and produce false positives; a positive screen is a reason to assess, not a label.
- Competence and copyright: use instruments you are trained to interpret; the Beck inventories are proprietary; the PHQ, GAD-7, AUDIT, and DSM-5-TR measures are free.
- Culture and language: use validated translations and consider norms; somatic idioms of distress may inflate or deflate scores; interpret in context.
- Documentation: record the instrument, date, score, interpretation, and the action taken (Domain 5).

What do BDI-II scores mean?
0–13 minimal, 14–19 mild, 20–28 moderate, 29–63 severe depressive symptom severity over the past two weeks. The score quantifies severity and tracks change; diagnosis requires a clinical interview.
What is the difference between the BAI and the GAD-7?
The Beck Anxiety Inventory emphasizes somatic and panic-related symptoms and was built to discriminate anxiety from depression; the GAD-7 captures generalized worry. Both are severity measures, not diagnoses; the GAD-7 is free and the BAI is proprietary.
What should a counselor do when a client endorses PHQ-9 item 9?
Conduct a direct suicide risk assessment immediately (ideation, plan, intent, means, history), ideally with a structured tool such as the C-SSRS, regardless of the total score. The item is a screen, not an assessment.
