3-Option Questions on the 2027 NCE: How to Read and Answer Them
By David Zimmerman, RN · 7 min read · Study strategy
Starting July 1, 2027, every NCE question has three answer choices instead of four. NBCC’s specifications for the new exam describe 170 multiple-choice questions with 3 possible responses each, of which 30 are unscored field-test items in the same format[1]. Losing an option changes the arithmetic of guessing and the feel of each item. It does not change what the exam measures. This post covers both, with five practice items at the end.
What does NBCC say about the 2027 question format?
- 170 multiple-choice questions, 3 responses each[1].
- 30 field-test items, randomly distributed and indistinguishable from the rest. Your score is based on the scored items only, which works out to 140[1].
- 225 minutes, with an optional 15-minute break halfway. You can review questions within each half before you submit it; after that, the half is closed[1].
- Scaled scores from 100–500, with 360 passing on every form. NBCC is explicit that the scale is not a percentage[1].
For comparison, the current NCE has 200 four-option questions with 160 scored[2]. The full side-by-side is in NCE 2027 changes explained.
How does guessing change with three options?
This is plain probability, nothing more. If you have no idea and pick at random, your chance of being right depends on how many options are left.
| Situation | Four options (current) | Three options (2027) |
|---|---|---|
| Blind guess | 1 in 4 (25%) | 1 in 3 (about 33%) |
| You rule out one option | 1 in 3 (about 33%) | 1 in 2 (50%) |
| You rule out two options | 1 in 2 (50%) | Only one left: you have the answer |
Two practical points follow. First, eliminating a single option is worth more on a three-option item: it takes you straight to a coin flip. Second, the current NCE handbook states there is no penalty for wrong answers[2], which is why the standard advice is never to leave an item blank. Confirm the guessing policy in the candidate materials in effect for your test date.
This is not the same as an easier exam
Why do the last two options often feel so close?
NBCC does not publish how it writes its three-option items, so no one outside NBCC can tell you how plausible its distractors will be. What is certain is the arithmetic: with two wrong answers instead of three, there is one fewer option to cross off before you are down to a choice between two. When we built our 2027 practice form, we removed each item’s least plausible distractor in an audited mapping, so in our sittings the obviously wrong choice you used to cross off first is usually not there, and you are left weighing the answer against a near miss. Practice for that case.
That raises the value of precise reading. When two options are both reasonable, the stem decides between them, usually through one word:
- FIRST, INITIAL, NEXT: both options may be things a counselor will do. Pick the one that must happen before the other, usually assessment or safety.
- BEST, MOST appropriate: both may be defensible. Pick the one most consistent with the client, setting, and ethics described.
- NOT, EXCEPT, LEAST: the task is reversed. Two options are true and you are looking for the odd one out.
- According to [a theory or code]: answer inside that frame, in its vocabulary, even if you would practice differently.
Our NCE test-taking strategies post covers each qualifier in more depth, with sample items.
A reading routine for three-option items
- Read the last sentence of the stem first. Find the qualifier and what is being asked: a step, a stage, a theory, an ethical duty.
- Read the scenario for the deciding detail. Age, setting, a stated risk, a named theory, what has already been done.
- Predict before you look. Say the answer in your own words, then read all three options.
- Eliminate with a reason. Say why one option is wrong. If you cannot, you are not ready to choose.
- Decide between the final two using the qualifier. Ask which one the stem’s key word actually rewards.
- Note it and move on if stuck. You can review within the current half before you submit it[1], so you can come back to it before that half closes.
How should you practice for the 2027 format?
Practice timed and in the real format, because three options change your rhythm. With 170 questions in 225 minutes, an even pace is about 79 seconds per item, compared with about 68 seconds on the current 200-item form. Each half of 85 items comes to roughly 113 minutes at that pace. The extra seconds are useful, but they are best spent on the items where two options are close, not spread evenly.
What to look for in your results
- Misses where you had it down to two. Go back to the stem and find the word that should have decided it. These are reading errors, and they are fixable.
- Misses where you had no idea. These are content gaps. Study the topic, not the item; the free topic pages on suicide risk assessment, professional ethics, and assessment are good places to start.
- Time per item by half. If the second half runs slower, practice full-length, not just short sets.
The CaseSavvy 2027 NCE practice sitting uses the format described above: 170 items with 3 options, 140 scored plus 30 unscored, two halves of 85 with a break after item 85, review limited to the current half, and a pacing budget of about 79 seconds per item. The 2027 NCE practice test post has six playable three-option samples, one from each 2027 domain.

Frequently asked questions
How many answer choices does the 2027 NCE have?
Three. NBCC's specifications for the NCE effective July 1, 2027 describe 170 multiple-choice questions with three possible responses each. Thirty are unscored field-test items in the same format, so 140 are scored.
Does having three options make the 2027 NCE easier?
Not by design. A blind guess has a 1-in-3 chance instead of 1-in-4, but NBCC sets the passing standard by having experts review each item as written, equates later forms, and reports a criterion-referenced result. The standard accounts for the three-option format.
How much time do I have per question on the 2027 NCE?
The 2027 NCE gives 225 minutes for 170 questions, which is about 79 seconds per item at an even pace. The current NCE gives 225 minutes for 200 questions, about 68 seconds per item.
What is the best strategy for three-option questions?
Read the last sentence of the stem first to find the qualifier (FIRST, BEST, MOST, NOT), predict the answer, eliminate one option with a reason, then decide between the final two using the qualifier. Ruling out one option on a three-option item leaves a 50/50.
